Susan A. Coates, LPN, was recently named clinical nurse liaison for Masonicare at Newtown and Masonicare Home Health & Hospice, both affiliates of Wallingford-based Masonicare, Connecticutâs leading not-for-profit provider of senior healthcare and retirement living.
In her new role as clinical nurse liaison, Ms Coates will work directly with hospital case managers in the Danbury and Waterbury areas to assess individuals for referral to Masonicare at Newtown and to Masonicare Home Health & Hospice, which serves the area from its Brookfield office.
She will also follow up with home care patients and facilitate interaction with area physicians to better meet patientsâ needs and enhance the level and quality of care they receive.Â
A resident of Middlebury, Ms Coates has several years of health care and administrative experience working in hospital, long-term care, and short-term rehabilitation settings. She attended McGill University in Montreal, Canada, and holds an LPN degree from Kaynor Technical College of Waterbury.
Ms Coates is a member of the Junior League, a Connecticut Junior Republic volunteer, and is a Connecticut Level 4 swimming coach. She and her husband, Richard, and their children live in Middlebury.
Masonicare at Newtown is a 154-bed skilled nursing and rehabilitation facility on Toddy Hill Road in the Sandy Hook section of Newtown. Masonicare Home Health provides home health and hospice care in Connecticut with several offices throughout the state.
