w Physical Form – Each member of a Newtown High School team must have a complete physical examination before he/she can be allowed to try out for a sport. The physical must be documented ONLY on the yellow health form and is good for one year from the date it was signed by the doctor. These yellow health forms can be found in the nurse’s office, the athletic director’s office, and in the main office of Newtown High School.

w Parent Permission Form – Each member of a Newtown High School team must have a complete parent permission form before he/she can be allowed to try out for a sport. This form is located on the back of the yellow health form and must be completed and signed by a parent for each new sports season.

w Pay To Participate Fee – Each member of a Newtown High School team must submit a pay-to-participate fee at the start of each new sports season. Checks should be made out to Newtown High School. The fee is used to help offset the cost of the sport and in no way guarantees a set amount of playing time. The fee is only refundable if a player is not selected for a team.
